Job Description

Sherrington Park Medical Practice is a 2 partner high achieving practice providing excellent patient care. We have a list size of c5000 and are a CQC “Outstanding” rated practice. We are based close to Nottingham City Centre with excellent transport links. We are looking for a lead nurse who will provide clinical and professional leadership to the nursing team at Sherrington Park Medical. The post holder will work as part of the practice multidisciplinary team, and will ensure nursing services are delivered effectively to the patient population.

The Lead Nurse is responsible for leading and directing the development of new services and patient care pathways, ensuring they match the needs of patients. The following are the core responsibilities of the lead practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

Responsibilities

As the Clinical lead, you will have overall line management responsibility for the Practice Nursing Team:

  • Provide clinical leadership to the nursing team
  • Mentor new nursing staff
  • Support the development of others to maximise staff potential in the team.
  • Identify the potential for service developments, risk and deficits and inform practice manager making recommendations based on specialist knowledge
  • Undertake one-to-ones, appraisals, regular Practice Nurse and HCA meetings, clinical education, and other practice-wide learning events and meetings.
  • To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Play a key role in our clinical governance and overall practice management team.
  • Act as a clinical leader in the delivery of practice nursing services to patients, ensuring that the needs of the patient are the priority.
  • Attend regular nursing meetings.
  • Lead the nursing team in the planning and implementation of local guidelines, protocols, and standards, as well as local projects or initiatives.
  • Ensure health and safety and infection control are high priorities, supported by other members of the team.
  • Work with our Practice Management team to ensure we monitor practice performance of QOF and enhanced services regularly to achieve high standards of patient care.
  • Take responsibility for own development, learning, and performance, including organising clinical supervision where appropriate and acting as a positive role model.

  • Work with colleagues and management team to ensure that we focus our clinical time to maximum benefit of patients and that our systems and processes support staff to focus on what matters most.
  • To oversee the provision of wound care services to patients
  • To co-ordinate the provision of travel medicine services
  • To process pathology results as required
  • To maintain accurate clinical records in conjunction with existing legislation
  • Act as infection control lead for the practice
  • To ensure read codes are used effectively by all members of the nursing team.
  • To chaperone patients where necessary
  • To prioritise health issues and intervene appropriately.
  • To support the team in dealing with clinical emergencies
  • To implement vaccination programmes for adults and children
  • To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • To support the clinical team with all safeguarding matters in accordance with local and national policies
  • To deliver opportunistic health promotion where appropriate.
  • To be responsible for ensuring maximum optimisation of use of appointments across the nursing team.
  • Develop and review audit protocols and process for the practice, effectively utilising the audit cycle and to oversee those within the team”
